A. Contract Registration Requirements (2 sets)
- Employment contract signed by the worker and the employer
- Copy of passport data page with at least six months validity from the intended date of departure from the Philippines
- Copy of residence permit showing the work visa with a validity period that would cover date of return to job site
- Copy of latest pay slip or company ID or Certificate of Employment
- Affidavit of the worker stating the circumstances and timeline on how employment was secure. The form can be downloaded here.

OWWA Membership Registration Requirements
The Overseas Workers Welfare Administration works for the protection and promotion of the welfare and interest of overseas Filipino workers (OFWs) and their families. The agency has twin mandate of delivering responsive welfare services and ensuring capital build-up and fund viability.
The OWWA membership fee is GBP 23.00 and the validity is 2 years.
Requirements:
- Filled out OWWA OFW Information Sheet
- Photocopy of the data page of valid Philippine passport
- Residence Permit (for work visa holders and permanent residents)
- Certificate of Oath of Allegiance, for dual citizens
- Proof of Employment (Employment Contract/ Latest Pay Slip/ Certificate of Employment/ Company ID)
