Application for Retention/Reacquisition of Philippine citizenship may only be applied for online. Proceed to online application.

 

Requirements:

  1. Personal appearance       
  2. Duly accomplished petition/application form (4 originals)  Please download here. 
  3. Four (4) pieces 2x2 photograph with white background taken within the last three months
  4. Original and three (3) photocopies of PSA-authenticated birth certificate
  5. Original and three (3) photocopies of valid foreign passport
  6. Original and three (3)  photocopies of certificate of naturalization or other naturalization documents with English translation legalized by the Danish MFA 
  7. For Philippine Bureau of Immigration-registered aliens ONLY
    1.  Original and three (3) photocopies of ACR I-Card or Alien Certificate of Registration; and
    2. Original and three (3) photocopies of Immigrant Certificate of Residence (ICR) or Certificate of Residence for Temporary Visitor (CRTV) 
    3. Affidavit of Loss and three (3) photocopies  if 7.1 or 7.2 are lost. 
  8. For married female who wishes to use spouse's last name in their Philippine passport - original PSA-issued marriage contract or Report of Marriage issued by the Philippine Embassy in Copenhagen.
  9. Other documents (e.g. cancelled Philippine passport) (if required).
  10. Service and expedited fee of DKK480.00 in exact amount, in Danish kroners only. Payment via instant bank transfer preferred. Account details will be provided at the Embassy processing counter on the date of your appearance.

CHECKLIST OF REQUIREMENTS FOR EACH DEPENDENT CHILD

  1. Duly accomplished Supplement for Dependent Form. Please download here. 
  2. Four (2) pieces 2x2 photograph with white background and must be taken within the last three months from the date of application;
  3. Original and three (3) photocopies of Birth Certificate of dependent child in English legalized by the Danish MFA 
  4. Original and three (3) photocopies of valid foreign passport bio-page of dependent child 
  5. For dependent children who are Philippine Bureau of Immigration-registered aliens:
         a. Original and three (3) photocopies of  ACR I-Card or Alien Certificate of Registration; and
         b. Original and three (3) photocopies of Immigrant Certificate of Residence (ICR) or Certificate of Residence for Temporary Visitor (CRTV)
         c.     Affidavit of Loss and three (3) photocopies  if 5.1 or 5.2 are lost. 
  6. Service and expedited fee of DKK280.00 in exact amount, in Danish kroners only. Payment via instant bank transfer preferred. Account details will be provided at the Embassy processing counter on the date of your appearance.
