Notarization of documents before a Philippine consular officer for use in the Philippines (Red Ribbon)

1. General affidavits
2. Acknowledgment of Special Power of attorney (SPA), contracts and other documents for use in the Philippines; acknowledgment that person/s signed document of his/her/their own free wil and deed
3. Authentication of documents - E.g. documents issued locally in Denmark such as business permits, school records or diplomas, documents consularized by local authorities, bank statements and others; for use in the Philippines. Please note that in general, documents issued and notarized in Denmark and apostillized or legalized by the Danish Ministry of Foreign Affairs (MFA) on or after 14 May 2019 NEED NOT BE AUTHENTICATED (OR ATTACHED WITH A RED RIBBON) by the Philippine Embassy. 
4. Affidavit of parental consent for unaccompanied travelling minors - Requirement for DSWD travel clearance for Filipino minors travelling from the PH to a foreign country unaccompanied by any of the parents or persons having parental authority or legal custody over the child.
5. Affidavit of parental consent for Waiver of Exclusion Ground (WEG) - Requirement of Philippine Bureau of Immigration for non-Filipino children below 15 years old who are unaccompanied by or not coming to a parent
6. Affidavit of invitation and financial support - may be required by the Danish Embassy when Filipino is applying for a Schengen passport or by the Philippine Bureau of Immigration upon departure of Filipino from the Philippines

REQUIREMENTS FOR ALL NOTARIAL SERVICES:

• Personal appearance
• Original and photocopy of passport data page 
• Service and expedited fee of DKK280.00 per document per copy, in exact amount, in Danish kroners only. Payment via instant bank transfer preferred. Account details will be provided at the Embassy processing counter on the date of your appearance.

2. Acknowledgment of SPAs, contracts and other documents
   1. Original and photocopy of document/s to be acknowledged 
   2. Printed passport data page or any valid government-issued ID of the authorized representative in the Philippines (for SPA only)
   3. Other documents (if required)
3. Authentication of Documents
   1. Original and photocopy of the document/s to be authenticated in English notarized by notary public and legalized by Danish MFA
   2. Other documents (if required)
4. Affidavit of Parental Consent for Unaccompanied Traveling Minors 
   1. Duly accomplished Affidavit of Parental Consent for Unaccompanied Traveling Minors form.  Please download here.
   2. Photocopy of data page/s of passport of child/children 
   3. Photocopy of birth certificate/s of child/children authenticated by the Philippine Statistics Authority (PSA) 
   4. If father is the appearing parent, photocopy of marriage certificate of parents authenticated by the PSA
   5. Photocopy of passport of accompanying parent or traveling companion 
   6. Other documents (if required)
5. Affidavit of Parental Consent for Waiver of Exclusion Ground (WEG) 
   1. Duly accoplished Affidavit of Parental Consent for WEG.  Please download here. 
   2. Photocopy of data page of passport of child/children 
   3. Birth certificate of child translated in English notarized by notary public and legalized by Danish MFA 
   4. Marriage contract of parents translated in English notarized by notary public and legalized by Danish MFA 
   5. Photocopy of passport of accompanying parent or traveling companion 
   6. Other documents (if required)
6. Affidavit of Invitation and Financial Support 
   1. Duly accomplished Affidavit of Invitation and Financial Support form.  Please download here. 
   2. Proof of financial capacity of invitor (e.g. employment contract, employment certificate, pay slip, bank statement) 
   3. Passport of invitee 
   4. Round trip plane ticket from the Philippines 
   5. Other documents (if required)

Note: Please bring all requirements including the requisite photocopies when submitting documents.  There are no photocopying services in the Embassy.
