I. ADULTS 18 years old and above
A. New passport
- Duly accomplished application form. Download form here.
- Original PSA-authenticated birth certificate OR Report of Birth issued by the Philippine Embassy in Copenhagen
- Any one of the following valid Philippine-government issued IDs: SSS/GSIS card; LTO driver’s license; PRC ID; OWWA E-card; COMELEC Voter’s ID; PNP Firearms License; PH Senior Citizen ID; School ID
- Service fee of DKK480.00 in exact amount, in Danish kroners only. Payment via instant bank transfer preferred. Account details will be provided at the Embassy processing counter on the date of your appearance.
B. Renewal of passport
- Duly accomplished application form (download here)
- Current passport
- Service fee of DKK480.00 in exact amount, in Danish kroners only. Payment via instant bank transfer preferred. Account details will be provided at the Embassy processing counter on the date of your appearance.
C. Lost, mutilated and/or damaged Passport
- Duly accomplished application form (download here).
- Current passport or photocopy of data page (for lost passport, if available). For lost passport and if photocopy not available, please submit original PSA-issued birth certificate.
- Any one of the following valid Philippine-government issued IDs: SSS/GSIS card; LTO driver’s license; PRC ID; OWWA E-card; COMELEC Voter’s ID; PNP Firearms License; PH Senior Citizen ID; School ID; host government-issued IDs showing Philippine citizenship (residence card)
- Service fee of DKK1,480.00 and additional fees if applying for additional services. Payment via instant bank transfer preferred. Account details will be provided at the Embassy processing counter on the date of your appearance.
- For lost valid Passport
- Police report in English
- Affidavit of loss (please download form here)
- For lost expired Passport
- Affidavit of loss (please download form here)
- For mutilated and/or damaged passport
- Affidavit of explanation (please download form here)
ADDITIONAL REQUIREMENTS FOR SPECIAL CASES:
- For married female who wishes to use spouse’s last name – original PSA-issued marriage contract (Report of Marriage issued by the Philippine Embassy in Copenhagen, less than a year ago may be submitted alternatively)
- For female whose marriage was annulled by courts and wishes to use maiden name – annotated original PSA-issued marriage contract
- For widow who wishes to use maiden name -
- Original PSA-issued death certificate of deceased spouse AND
- Original PSA-issued marriage contract
- For applicant who reacquired or retained their Philippine citizenship as specified in RA 9225 - Original and photocopy of Identification Certificate, Order of Approval and Oath of Allegiance issued by a Philippine Foreign Service Post (FSP) or by the Philippine Bureau of Immigration
- For applicant who is a Naturalized citizen - Original and photocopy of Identification Certificate of Naturalization from the Philippine Bureau of Immigration
II. MINORS (below 18 years old)
- Duly accomplished application form (download here)
- Passport of accompanying parent or authorized adult companion
- If father is the accompanying parent, marriage contract
- If unaccompanied by either parent – letter of authority executed by parent/s designating the companion to assist in the application process
- For new passport – Original PSA-issued birth certificate OR Report of Birth issued by the Philippine Embassy in Copenhagen
- For passport renewal
- Current Passport
- Proof of filiation (any one of the following documents: PSA-issued birth certificate; baptismal certificate; DSWD travel clearance)
- Additional requirement for married and unmarried parents of minor: Valid ID or passport copy of father.
- Service fee of DKK480.00 in exact amount, in Danish kroners only. Payment via instant bank transfer preferred. Account details will be provided at the Embassy processing counter on the date of your appearance.
