Vital or civil registration is the recording in the appropriate civil registers, vital acts and events that affect the civil status of individuals. Vital acts and events are the births, deaths, fetal deaths, marriages, and all such events that have something to do with an individual’s entrance and departure from life together with the changes in civil status that may occur to a person during his lifetime.

The Philippine Embassy in Copenhagen performs civil registry services for vital acts and events (births, marriages and deaths) of Filipinos which occurred in Denmark.

SERVICES:

  1. Report of birth
    • To establish the birth, citizenship, parentage, legitimacy and other details of a Filipino child born abroad
    • Must be reported within 1 year after birth of child
  2. Report of marriage – must be reported within 1 year after the solemnization of marriage
  3. Report of death – must be reported within 1 year after date of death 

OTHER SERVICES:

  1. Certificate of legal capacity to contract marriage (CLCCM)
  2. Solemnization of marriage

GENERAL REQUIREMENTS:

  • Personal appearance
  • Original and 4 photocopies (per service) of passport data page
  • Service and expedited fee of DKK280.00 per service and additional DKK280.00 for additional documents, in exact amount, in Danish kroners only. Payment via instant bank transfer preferred. Account details will be provided at the Embassy processing counter on the date of your appearance.
  • Unless otherwise stated, please bring original and 3 photocopies of ALL documents (1 copy each for the Philippine Statistics Authority (PSA), Department of Foreign Affairs (DFA), Embassy and applicant)

REQUIREMENTS FOR SPECIFIC SERVICES:

  1. Report of Birth
  • Passport of mother
  • Passport of father
  • Duly accomplished Report of Birth form (4 original copies; preferably typewritten);
  • Certificate of birth issued by Danish local authorities in English AND LEGALIZED BY THE DANISH MINISTRY OF FOREIGN AFFAIRS 
  • PSA-issued marriage contract of parents OR Report of Marriage issued by  the Philippine Embassy in Copenhagen
  • Affidavit for delayed registration (for registration beyond 1 year after birth)
  • For unmarried parents:
    • Affidavit of admission of paternity, if applicable
    • Affidavit to use the surname of the father, if applicable

      2.  Report of Marriage

  • Passport of wife
  • Passport of husband
  • Report of Marriage form executed by parties (4 original copies; preferably typewritten); entries must be at THE TIME OF MARRIAGE of the spouses
  • Marriage certificate issued by Danish local authorities in English notarized by a notary public AND LEGALIZED/APOSTILIZED BY THE DANISH MINISTRY OF FOREIGN AFFAIRS (except if marriage solemnized at Embassy; Entries must be at the time of marriage of the spouses).
  • If one or both parties was/were previously married
    • For annulled Filipino spouse -  marriage contract issued by the PSA with proper annotation
    • For divorced Filipino spouse -  divorce decree/certificate by the Regional Trial Court OR marriage contract issued by the PSA with proper annotation (original and 3 photocopies)
    • For widowed Filipino spouse - death certificate issued by PSA or foreign death certificate of previous spouse AND marriage contract with previous spouse
    • For divorced or annulled foreign spouse - foreign decree/decision with English translation (4 photocopies)
    • For widowed foreign spouse - death certificate with English translation of previous spouse (4 photocopies) 
  • Affidavit for delayed registration - for registration beyond 1 year after marriage; 1 original and 3 photocopies

 

      3.  Report of Death

  • Duly accomplished Report of Death form (4 originals; preferably typewritten)
  • Passport of deceased
  • Accounting of personal effects of deceased
  • Certificate of Death in English notarized by notary public and legalized by the Danish MFA  
  • Affidavit of Delayed Registration of Death (if the Report of Death is filed more than 30 days after demise of the deceased).
  • IF REMAINS ARE TO BE TRANSPORTED TO THE PHILIPPINES
    • Certificate of Non-contagious Disease issued by the proper health authority in English and notarized (original and 3 photocopies)
    • If cremated:
      • Certificate of Cremation in English notarized by notary public and legalized by Danish MFA (original and 3 photocopies)
      • If to be sent to the PH as unaccompanied cargo
        • copy of airway bill and flight details (original and 3 photocopies
        • Consignee’s complete contact details (address, phone numbers, and email address; 4 copies)
      • If to be checked-in by accompanying person:
        • flight details of accompanying person (4 copies)
        • copy of passport of accompanying person (4 copies)
    • If embalmed:
      • Embalming Certificate in English notarized by notary public and legalized by Danish MFA (original and 3 photocopies)
      • Mortuary Certificate (original and 3 photocopies)
      • Certificate of Sealing of Casket (original and 3 photocopies)
      • Corpse Transmit Permit (original and 3 photocopies)
      • Copy of airway bill and flight details (4 copies)
      • Consignee’s complete contact details (address, phone numbers, and email addresses; 4 copies)

4.  Certificate of Legal Capacity to Contract Marriage CLCCM)

  • Application for CLCCM
  • Certificate of No Marriage authenticated by the Philippine Statistics Authority (PSA) and DFA (original and photocopy)
  • Other documents (if required; original and photocopy)
    • If previously married: death certificate or judicial decree of absolute divorce/judicial decree of annulment or declaration of nullity; annotated marriage contract (original and photocopy) (Authenticated by PSA and DFA)
    • If 18-20 years old: parental consent
    • If 21-25 years old: parental advice
    • If 18-25 years old: certificate of marriage counseling
    • Others

5.  Solemnization of Marriage

  • Application for marriage license (original and photocopy)
  • Passport size photo picture
  • Certificate of no marriage authenticated by the Philippine Statistics Authority (PSA) (original and photocopy)
  • Other documents (if required; original and photocopy)
    • If previously married: death certificate or judicial decree of absolute divorce/judicial decree of annulment or declaration of nullity; annotated marriage contract (original and photocopy) (authenticated by PSA)
    • If 18-20 years old: parental consent
    • If 21-25 years old: parental advice
    • If 18-25 years old: certificate of marriage counseling
    • Others
  • 10 days notice and publication;
  • At least 2 witnesses of legal age;
  • Marriage ceremony
  • Service and expedited fee of DKK480.00
    TO BE ISSUED BY THE EMBASSY:
  • Notice of Marriage (publication for ten days)
  • Marriage license (valid for 120 days)
  • Marriage certificate
  • Report of marriage 

Note: Please bring all requirements including the requisite photocopies when submitting documents.  There are no photocopying services in the Embassy.
